This form is consent for C3 New Hope's ministry activities and also so that we can let you know what is happening at youth and inform you of anything you need to know.

Consent

Please select 'yes' to give your consent to the following items and your overall permission for your child/ward to participate in ministry activities held by C3 New Hope.
In some circumstances additional permissions will be required. In this case the nominated parent/legal guardian will be contacted with further detail.

Privacy:

You confirm that you have read and agree to the collection of you and your child's personal information including sensitive information in accordance with C3 New Hope's Collection Notice and Privacy Policy.

Photography:

You understand that photographs are taken during ministry activities for the purpose of Church communications and promotion such as newsletters and Church social media. C3 New Hope respects that not everyone would like photographs of their child used for this purpose. If this is the case, please email compliance@c3newhope.com

General Consent:

- I give permission for my child to participate in the activities & operations of C3 New Hope, including the skate park.
- I am aware of the risks involved in the activity that my child is participating in and that there may be inherent risks associated with activities provided.
- I agree that participation is voluntary.
- I understand that children must follow the instructions of supervisors at all times.
- I understand that in the case of any situation in which my child fails to comply with this agreement, the child may be required to no longer participate in the activity.
- I understand that failure of my child to follow instructions will require the parent or legal guardian to pick up their child from the venue and that I am responsible to pick up my child from the venue.
- I understand that if my child becomes ill and needs to go home, I am responsible to pick up my child from the venue.
- I agree to pay to the Church costs detailed above for participation in the activity.
- I understand that it is the parent's/guardian's responsibility to provide transport for the child in relation to all activities provided by C3 New Hope. 
- I agree that in the event of my child being involved in an accident, becoming ill, or otherwise requiring medical treatment or care, C3 New Hope or its personnel, volunteers or supervisors may, in their absolute discretion, obtain medical treatment or an ambulance for the child and that I must pay all expenses incurred in obtaining such medical treatment or care.
- I agree that C3 New Hope, workers and volunteers of the provider, and other participants are not responsible for any medical insurance, coverage or care in the event of illness, accident or injury.
- I agree that my child is responsible for any fines or penalties incurred in respect to their activity.
- I understand that to the extent permitted by law, neither C3 New Hope nor its directors, employees or volunteers accepts any liability for any loss, including consequential loss, damage, accident, death or injury incurred from this activity or agreement.
- I understand that C3 New Hope is not responsible for my child consuming foods that they are allergic to.  
- I understand if my child has initiated a social media connection with a volunteer or leader that these platforms may be used to communicate with my child. Procedures in relation to communicating via social media can be found in C3 New Hope's Child Protection Policy. 
- I hereby indemnify & release C3 New Hope and each of its participants, members, volunteers & leaders against any and all losses, costs, damages, expenses and liabilities (including legal costs on a full indemnity basis) sustained or incurred by C3 New Hope or any of its personnel, participants, members, volunteers & leaders incurred from this activity or agreement whilst on site or travelling to and from the venue.

Please read carefully and communicate these expected behaviours to your child or children. By signing you are acknowledging that you and your child or children understand these expectations and agree to abide by them.

Respect for Others

   - Be polite, kind, and considerate to fellow children and youth leaders

   - Treat others with respect, embracing diversity, and showing consideration for others

   - Use positive language and avoid using hurtful or offensive comments.

Respect for Property

   - Take care of the space and facilities provided for activities, including chairs, tables, and equipment.

   - Keep the area tidy and clean up after yourself, including disposing of trash properly.

Appropriate Behavior During the Service and Following Instructions

   - Listen carefully to and follow the instructions of youth leaders and volunteers

   - Stay engaged during the service avoiding being disruptive to others

   - Be respectful during worship, prayer, and other moments throughout the service as well as listening to the instructions given by Youth Leaders and volunteers.

Self-Control and Conflict Resolution

   - Show self-control, especially during group activities, and manage emotions appropriately.

   - If there’s a disagreement with a peer, resolve it calmly and respectfully, seeking help from a leader or volunteer if needed.

   - Use kind words and actions when resolving conflicts. Do not use any means of physical violence to resolve conflict.

No Bullying 

    - Bullying, teasing, or any form of disrespectful behavior is not tolerated.

    - Be supportive and inclusive of all children, not discriminating based upon belief or cultural background

Safety and Well-Being

    - Follow all safety guidelines provided by leaders and volunteers, especially when moving around the building or during group activities.

    - Report any accidents, injuries, or unsafe situations immediately to a leader or volunteer or the First Aid Officer

Electronic Devices

    - Use electronic devices (phones, tablets, etc.) appropriately not filming in inappropriate areas such as bathrooms.

    - Do not misuse phones or other devices to cause harm to others
